Ann Street Family Medicine was established in 1992 by Dr Mark Veltmeyer and original co-founders of the practice. Since that time, the practice has grown to its current size with a full complement of 8 doctors and 2 registrars. We are situated next door to QML Pathology on our left and the Friendlies Pharmacy is next door on our right.
Ann Street Family Medicine have always been committed to promoting health, wellbeing and disease prevention to all patients in the local community. Due to our long period of service as Ann Street Family Medicine, the clinic has been able to offer the highest possible continuity of care. Our patients have been able to establish a rapport with their own personal doctor over a long period of time.
Due to the changes within the medical environment, our practice has grown and changed in the way we care for our patients needs and we have expanded in all areas of medicine and are able to offer our patients a greater range of services. We now employ a full time care plan nurse and have 7 other practice nurses that perform various duties. We are available to our patients 7 days a week for their health concerns.
Our doctors strive to provide the highest standard of patient care to people in the local area and surrounds. Our doctors and staff do not discriminate in the provision of excellent care and aim to treat all patients with dignity and respect.

Test Results, Repeat Prescriptions and Referrals
The preferred method of obtaining test results is to book a further consultation with your general practitioner. If you doctor has any concerns regarding test results, you will be contacted by phone. It is therefore extremely important that you advise the staff of any changes to your details.
In the interest of optimum patient care and safety, doctors at Ann Street Family Medicine do not dispense prescriptions for medication without a consultation.
When requesting referrals, it is appropriate to see your doctor in the first instance so your doctor can re-assess your condition and its urgency and if necessary write a referral to the most appropriate specialist.
Phone calls are taken by our Doctors during consultation if the caller is another Doctor, Specialist, Pharmacist or in an emergency. In the event of all other calls, a message is taken and delivered to the Doctor.
Management of your Personal Health Information
This practice has a policy to maintain the safety and securities of personal health information at all times and ensure that this information is only available to authorised staff members and only used to provide accurate medical diagnoses and appropriate treatment.
